Accident Investigation

This session is designed to help the practitioner understand accident investigation from the HSE’s perspective and why things don’t always turn out as you might expect. It covers the thorny subject of blame and how just slight changes to the process of the organisation under investigation may help improve overall outcomes
In this session, the delegates will learn: 
  • How the enforcing authority (principally the HSE) views the accident investigation process
  • What organisations need to do to prepare themselves for any enforcing authority involvement
  • About the Accident Investigation Protocol and why it is the key to ensuring a good outcome from an accident investigation
  • What the implications are of the approach taken to accident investigation for an organisation, their stakeholders and wider society 
About the speaker
Mike Flannery has been involved in managing risk since 1980 initially as an underwriter, and then as a broker assisting a wide variety of clients to transfer their risks successfully. Since 1992 Mike has been involved in prevention of risk rather than the transfer.  He is a Chartered Member of IOSH and a member of the Institute of Industrial Accident Investigators (IIAI). Mike has been involved in this work as a practitioner and as a specialist accident investigator, most recently in the areas of workplace transport, construction and asbestos.
